Letter/Email Request - New Client
Select if NOT ALREADY a Current Client
Service Description
Request a formal letter or email be written that documents the agreed upon information from communication with Dr. Grant, such as suggestions regarding ideas for supports/accommodations, possible diagnostic implications, potential explanations for child behaviors, etc. This letter will not be written in conjunction with information specific to your child. It will offer information and/or strategies related to best practices, research, and/or insights that might be helpful for someone to better understand or work with any child that meets that criteria. As an example, it might begin as follows: Jane Doe has contacted me to request I provide information related to Fetal Alcohol Spectrum Disorders that may be relevant and helpful for her son, John Doe, whom she has reported has been given this diagnosis. I hope the information I provide below, as well as the resources I have included, are helpful to you and John's educational team when you combine them with the information and observations you already have about this student and the accommodations that may best help him find success in your district. When booking this service, please pick an available time and day closest to when you need the letter completed by. In other words, think of the scheduled appointment as the "due date" though Dr. Grant will be in contact with you prior to that time.
Important Information
1) Scheduling a consultation with Dr. Grant is significantly different from seeing Dr. Grant as a therapy client. Consultation work is intended to be fairly brief and is less "intense" or in depth than therapeutic services often are. Discussions will often be educational and supportive, offering recommendations for next steps, referrals, or resources as opposed to spending time processing and exploring patterns that may support change. 2) While the office works to maintain confidentiality around all the work we do with families, the same legal protections do not apply to consultation services as for therapeutic services. HIPAA is not required for these services and legal requests for records are not given the same protections. Mandated reporting laws do continue. 3) You will be invoiced for this service following its completion. Connected Development, PLLC does not accept any insurances at this time and this service is unlikely to be reimbursable by insurance, however, you may retain your receipt for tax purposes. If you plan to use an HSA or Flex Account, please let Dr. Grant know so she can issue your invoice to accommodate that method of payment. 4) Missed appointments that were not cancelled with 24 hours notice may be billed a missed appointment fee of $55. 5) Regarding court involvement...you or the attorney in writing will be charged a $1500 retainer due in advance should Dr. Grant be subpoenaed to engage in legal matters regarding individuals related to this consultation. It is highly recommended that Dr. Grant be made aware of any possible legal situations in advance or as they arise so they can be discussed and handled in a way that best supports those involved. Should you be interested in discussing having Dr. Grant act as an expert witness or engage with legal matters, please request information to review for this service.
