The "Fix it Form" is an alternative to the "Think Sheet" and was created specifically to use in an elementary classroom setting. It takes a more brain-based approach by recognizing that yuck behaviors from kids are often not intentional or in their control, but it still allows there to be a level of accountability and repair.
Students would be walked through (or could complete on their own) the first two pages, somehow indicating which options fit them best. The third page is an optional (note that word) sample apology note that makes the repair a bit easier on a child who struggles with acknowledging what they did, often due to shame.
I do have to note that while I think this is a good alternative to the "Think Sheet", there are still students who will find this further dysregulating or shameful. No strategy fits every child's needs, so no strategy (even this one) should be required for every child - especially in moments of stress. Also, students don't need to stick with the choices offered!! If they have another idea or want to adjust something on the sheet - LET THEM. It's a great way to encourage self-exploration.
